There are three Frost Wurm Young in the last level of Frostmaw's Burrows. Approaching them causes Frost Wurm Larvae to pop up near them. Once you have killed these groups 3 Frost Siege Wurms, 2 Frostmaw Spawns, and Frostmaw the Kinslayer will spawn.
